ULTRASOUND GUIDED SCLEROTHEARPY:

Ultrasound Guided Sclerotherapy is the gold standard of modern day vein treatment and Willamette Vein Centre' was one of the first vein centers on the west coast to offer patients this advanced technique. Locating the highest point of reflux and treating that etiology with ultrasound guided sclerotherapy greatly improves, not only varicose veins, but by eliminating their source under the surface of the skin, vastly improves treatment results of reticular veins and spiders.

During their preceptorship attendees will observe first hand a wide selection of Ultrasound Guided Sclerotherapy treatment sessions performed on real patients of Willamette Vein Centre' and upon completion of their preceptorship attendees will be prepared to explore this exciting field of phlebology as it relates to their own practices when they return home.

SURFACE SCLEROTHERAPY – FOAM TECHNIQUE

Sclerotherapy is not a "point and shoot" procedure. Good phlebology necessitates a thorough understanding of venous anatomy & physiology, a working knowledge of sclerotherapy principles, and impeccable technique. All of this is taught in detail with every preceptorship.

Sclerotherapy technique: during their preceptorhip attendees will learn various tried-and-true techniques employed at Willamette Vein Centre' for the past 14+ years to treat reticular veins and spider veins.

Foam: we have found that "foaming" sclerosing agents dramatically reduces the number of treatments a patient requires and foaming improves results. Preceptors will learn the Frullini technique of foaming Sodium Tetradecol Sulfate (STS) and will learn how to employee this technique in ultrasound guided sclerotherapy and in surface treatment of reticular veins.

LASER

Don't be mislead by claims from laser companies that you should buy a laser and then be able to treat all aspects of vein disease while becoming wealthy in the process. It just isn't true!

We have several lasers at Willamette Vein Centre', and when used appropriately, LASER can be a valuable asset to a phlebology practice. But laser augments sclerotherapy – it does not replace it! LASERS are expensive to own, expensive to maintain, and it's almost impossible to amortize their cost.

During the preceptorship attendees will be introduced to the Aura KTP laser by LaserScope and may have the opportunity to use hands-on this valuable piece of technology themselves.

EVLT – EndoVenous Laser Treatment of the Greater Saphenous Vein & SFJ reflux

Willamette Vein Centre' was one of the first offices in the country to offer patients EVLT – EndoVenous Laser Treatment – a minimally invasive, ultrasound guided method of managing SFJ reflux and GSV disease by the introduction of a laser fiber into the Greater Saphenous Vein and ablating the vein with laser energy. Depending upon the type of preceptorship desired, attendees may have the opportunity to observe and actually assist with the EVLT procedure while attending their preceptorship.

AMBULATORY PHLEBECTOMY

For the surgically oriented phlebologist interested in expanding their level of expertise to including ambulatory stab-avulsion surgical management of varicose veins, preceptors may choose an advanced preceptorship in which techniques of ambulatory phlebectomy and tumescent anesthesia are demonstrated.

VENOUS ULTRASONOGRAPY

Even the beginner phlebologist needs to have a working knowledge of venous anatomy from an ultrasound viewpoint. Therefore, we feel it imperative that all preceptorships include both didactic and hands-on experience with venous ultrasonography.

Preceptorships will have hands-on experience with the ultrasound and will learn to recognize the all important "Mickey Mouse" sign of the sapheno-femoral junction. Preceptors will learn how to clinically determine reflux and the importance of incompetent perforators. Both the preceptor and his/her attending medical assistant will spend as much time as they wish with our office's venous ultrasonographer to accomplish this important educational goal.

Digital PPG – Digital Photo Plethysmography

Whereas venous ultrasonography is a study of venous anatomy, plethysmography is a study of venous physiology. Every intermediate to advanced phlebologist needs to understand the principles behind plethysmography and how to use this important technology in their practices. Depending upon the type of preceptorship desired, attendees will have the opportunity to perform and analyze digital photo plethysmography using the Elcat Digital PPG machine used at Willamette Vein Centre'.

VENOUS ULCER MANAGEMENT

As your phlebology practice grows you will be called upon to manage all types of venous disorders including venous ulcers. Depending upon the goal(s) of your preceptorship our method of treating venous ulcers with ultrasound guided sclerotherapy and compression will be discussed.

DIAGNOSTIC PHLEBOLOGY

An important standard of care at Willamette Vein Centre' is that all patients – from those having only small spider and reticular veins to those suffering from large varicose veins – receive a clinical venous examination and venous ultrasound before beginning treatment. We feel anything less does not give a complete picture of the patient's venous condition.

All preceptorships include a demonstration of our method of performing and recording our venous examinations (we give you all the forms and recording keeping materials we use in our practice) and we demonstrate how we use a brief, but very thorough, venous ultrasound examination on all new patients prior to their first treatment session.
